We ask each of our users to acknowledge receipt of our Notice of Privacy Practices. You acknowledge receipt of the Notice by Medviz. You can receive a copy of this notice by printing one from our website at any time. 

We take active steps to protect the privacy of your Protected Health Information ("PHI"). PHI includes information that we have created or received regarding your health or payment for your health. It may include both your medical records and personal information such as your name, social security number, financial information, address, and phone number. 


We work to: 

  • Protect the privacy of your PHI. All of our employees and providers are required to maintain the confidentiality of PHI and receive appropriate privacy training. 

  • Provide you with this Notice of Privacy Practices, explaining our duties and practices regarding your PHI. 

  • Notify you in case of a breach of unsecured PHI.

1. How We Use and Disclose Your PHI 

We use and disclose PHI in a number of ways connected to your treatment, payment for your care, and supporting healthcare operations, including, for example: 

  • For treatment purposes: Sharing PHI with physicians, nurses, and others involved in your healthcare or preventive healthcare.

  • Send you appointment reminders and other communications relating to your care and treatment. 

  • Let you know about treatment alternatives or other health-related services or benefits that may be of interest to you, via email, phone call, or text message. 

  • Contact you directly: We may use your PHI, including your email address or phone number, to contact you. For example, we may also use this information to send appointment reminders or other communications relating to your care and treatment, or let you know about treatment alternatives or other health-related services or benefits that may be of interest to you. 


We may also make certain PHI accessible to you through secured online tools. 

If you choose to communicate with us via email, text, or chat, you acknowledge that we may exchange PHI with you via those means. Please note that email, text, and certain chat functionality may not be secure methods of communication. By communicating with us in these ways, you agree to the security risks. If you would prefer not to exchange PHI via email, text, or chat, you can choose not to communicate with us using these means and can notify us at support@medviz.ai. 

2. What We Do Not Do 

It is important to know that: 

  • We do not sell your personal information for monetary value. 

  • We do not sell aggregated information for monetary value. 

  • We do not use your contact list. 

  • We do not track your device location. 

  • We do not track your browsing activity on other sites. 

  • We do not provide services to minors (users under 18). 

3. Use or Disclosure of PHI Without Authorization 

We may use or disclose your PHI without your authorization for legal and/or governmental purposes, including, for example: 

  • Sharing with oversight agencies: To health oversight agencies for certain activities such as audits, examinations, investigations, inspections, and licensure. 

  • In the course of legal proceedings or in response to an order of a court or administrative agency, subpoena, discovery request, or other lawful process. 

4. Modifications to This Notice 

We may modify this Notice of Privacy Practices from time to time. The revised Notice will apply to all PHI that we maintain. We will make any changes to this Notice by posting the revised Notice on our website. The date of the last update will be clearly indicated at the top of this Notice. Please review this Notice from time to time to ensure you are familiar with our privacy practices.
